JCA Parents and Students,
JCA, in partnership with Jewel Osco, is offering an opportunity for students to receive the flu shot by appointment only on Thursday, October 22 and Friday, October 23.
Please follow the link below to sign up for a flu shot at JCA. Students will need to bring in the completed waiver with insurance information to receive their shots. Waivers will be emailed to families after they sign-up and will be available on the JCA website.

3) Sign up! It’s Easy – you will NOT need to register an account or keep a password on SignUp.com.
Parents will receive an email with a waiver to be filled out on October 19th. Most insurance covers the cost ($32) of the shot, please check with your provider for coverage.
